If you cannot sign in to your MyDrury account to access your Form 1098–T, you may request one with this form. You may also call or email to request a form. The 1098–T form requires a social security number or another form of taxpayer identification number. If you have not provided Drury with your SSN, please contact the registrar to add it to your student information.
